As a patient of Alvis Chiropractic, you are entitled to receive a “Good Faith Estimate” detailing the expected costs of non-emergency healthcare services and items. This estimate is provided to patients who either do not have insurance or are choosing not to use insurance for their care. The estimate will include the total anticipated costs of services, such as chiropractic treatments, diagnostic tests, prescribed medications, and any related medical expenses.

To ensure you receive a timely estimate, it is important to request a written Good Faith Estimate at least one business day before your scheduled appointment. You may also seek estimates from other providers for comparison.

If the final bill exceeds the provided estimate by $400 or more, you have the right to dispute the charges. Be sure to retain a copy of your Good Faith Estimate for your records.
